Comments:
A renewal inspection was conducted on 3/5/24 from 10:25am to 12:30pm. There were 53 children present, with 9 staff supervising. School age children were present today and were observed watching a movie, Pre-K children were observed in small group activities and the preschoolers were observed completing a craft activity. The lunch menu included chicken taquitos, beans, corn, pineapples and milk. There were no medications on site. 3 staff records, 3 children's records, attendance records, injury reports, emergency drill logs and other center documents were reviewed to assess compliance.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use staff names; list staff by positions only. Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website. Description: Based on records review, the center did not obtain immunization documentation for a child in care, by the first day of enrollment.
Evidence:
1. Child # 1's first day of enrollment was 3/4/24.
2. Child # 1's immunization record has not been obtained to this date.

Description: Based on records review, staff records did not contain complete information.

Description: REPEAT VIOLATION:
Based on record review, observation and staff interview, attendance was not documented as it occurred.
Evidence:
1. At 10:25 am there were 11 children in the 4's classroom.
2. The attendance record showed that 13 children were signed in. The attendance record was not accurate.
3. Staff # 3 stated that 2 more children were expected and had been signed in on the attendance record.

Description: Based on records review, staff interview and observation, staff orientation for a new employee was not completed and documented prior to staff working alone with children.
Evidence:
1. Staff # 2 stated that Staff # 1 started working on 3/4/24.
2. Staff # 1 was observed working alone with the 3's classroom.
3. Orientation was not completed and the documentation was not on file for Staff # 1 prior to her working alone with the children.
