A monitoring inspection was initiated on March 5, 2024 from a self report ed incident received by the Fairfax Licensing Office on February 28, 2024.
The information gathered during the investigation does not support the allegation, so the complaint is determined to be ?not valid.? The written record of an injury for child #1, did not contain the required documentation of date and time when the parent was notified or how the parent was notified.Plan of Correction: I will make sure all the required documentation is recorded on the reports.
