22.1-289.011F The inspection summary and violation notice need to be posted in a conspicuous place.

Comments:
An unannounced monitoring inspection was conducted on 03/08/2024 from 9:20am to 11:20am. There were children ages 14 months to age 6 years old supervised by 7 staff. The physical plant, outdoor playground area, programming, attendance, menus, 3 staff records, 3 children?s records, 4 medications with corresponding authorization records, emergency drills, injury reports, emergency supplies, and policies were reviewed. Children were observed participating in snack, floor play, and fine motor activities. Diapering and handwashing procedures were also observed. There was an adequate number of staff present with current certification in Medication Administration Training (MAT), CPR and First Aid, and Daily Health Observation (DHO) training. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 8VAC20-780-60-A-8
Description: Based on review of 3 child records, the provider did not ensure they had a written care plan for each child with a diagnosed food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Evidence: The record for Child #1 did not contain an allergy care plan for their physician diagnosed food allergy.

Plan of Correction: The allergy plan has been requested by their physician and will be on record by 4/1/24.

Standard #: 8VAC20-780-510-E
Description: Based on review of 4 medications and medication authorizations, the center did not ensure they follow the policy for administration of medication that requires the duration of the parent authorization be renewed after 10 workdays and written medication authorization should be given from the child?s physician and parent for a long term medication.

Evidence:
For 2 medications for Child #2, parent authorization was dated 10-8-23 and had not been renewed, and no authorization was given from the physician.

Plan of Correction: Medication has been returned to parents.

Standard #: 8VAC20-780-510-L
Description: Based on observation, the center did not ensure that medication be kept in a locked place using a safe locking method that prevents access by children.

Evidence: During inspection in the Children?s House classroom (ages 2-6) in an unlocked cabinet there was a bottle of Ibuprofen medication that was stored inside a first aid kit that was unlocked.

Plan of Correction: The medication has been removed from the classroom completely.
