Comments:
A Monitoring Inspection was conducted today. A total of 22 children with 3 teachers, 3 co-op parents and 3 administration staff were onsite today. A total of 3 classrooms were occupied. All 3 classrooms were observed. The theme of the curriculum was "fire safety'". Several books, activities and art projects kept the children actively engaged. The classes took turns rotating to the art room where they were able to paint fire flames. The Explorer's B class was observed at the end of a hallway gathered in a circle while the lead teacher sang with a guitar and had the children participate with musical shaking eggs. The Inventor's B classroom were observed lining up to wash their hands before taking their seats for morning snack. A total of 4 children's files and 4 staff and co-op files were reviewed. The previous violations from the last inspection were reviewed and the corrective actions were verified today. This inspection occurred between 10:15am and 12:25pm. Standard #: 8VAC20-780-40-M
Description: Based on observation of the center, a written list of children's allergies was not kept confidential.
Evidence:
The allergy list located in the yellow classroom was hanging on a wall in the room and was visible.

Plan of Correction: We will relocate the list of allergies to ensure confidentiality.

Standard #: 8VAC20-780-245-J-3
Description: Based on review of medication, one child who has an emergency medication is not in the care of a staff member who meets the requirements of medication administration.
Evidence:
An Auvi Q epinephrine is onsite for one child. No staff have completed the additional Auvi Q training requirement.

Plan of Correction: At least one staff will be present at all times who have completed this additional medication training component.
