Technical Assistance:
LI had discussions with center management regarding sound machine placement for infants.

Comments:
Today an unannounced Monitoring Inspection took place between the hours of 11:45am and 1:30pm. There were 6 classrooms observed with a total of 65 children with 10 staff within the supervision guidelines. The children were observed eating lunch (fish sticks, mixed vegetables, pineapple and milk) and transitioning to naptime. A complete inspection of the physical plant, children and staff records and emergency procedures were observed during this inspection. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. Areas of non-compliance are identified on the Violation Notice.

Description: Based on record review and interview, the center did not obtain subsequent background checks for each employee every five years.
Evidence:

Description: Based on record review and interview, each employee has not completed a sworn disclosure statement prior to employment.
