Comments:
An unannounced mandated inspection was completed from 9:30am until 11:50 by two inspectors.
There were 43 children in care and 13 staff present. Five children?s records and five staff records were reviewed. Children were observed washing hands, eating, resting, and playing.

The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. The playground equipment is currently not being used until the mulch can be spread.

Information gathered during the inspection determined non-compliance with applicable standards or law and violation (s) were documented on the violation notice issued to the program. An exit meting was held with the director.

Violations:
Standard #: 22.1-289.011-F
Description: Based on observation and interview, the center failed to have posted the last violation notice.
Evidence:
1. The acknowledgement form was posted from the last inspection but not the inspection documents.

Plan of Correction: The most recent inspections will be posted from now on when they are received.

Standard #: 8VAC20-780-270-A
Description: Based on observation, the center failed to keep the center in a safe and operable condition.
Evidence:
1. In the classroom near the back door there was a piece of rubber floor trim that had detached from the wall and was laying in the floor.

Plan of Correction: The trim will be reattached.

Standard #: 8VAC20-780-380-A
Description: Based on observation and interview, the center failed to have a daily schedule posted in two classrooms.
Evidence:
1. Two classrooms did not have schedules posted and the director verified this.

Plan of Correction: Schedules will be posted.

Standard #: 8VAC20-780-430-K
Description: Based on observation, the center failed to have an individual place for each child's personal belongings.
Evidence:
1. A classroom had children's belongings stored in the hallway near the back entrance.

Plan of Correction: A cubby system has been ordered. Clarification will be received from the fire marshal as to the best location to put them.

Standard #: 8VAC20-780-500-B
Description: Based on observation, the center failed to have a nonabsorbent surface for diapering and changing.
Evidence:
1. The diapering pad in the hallway had two tears in it.

Plan of Correction: Diaper pads have been ordered.

Standard #: 8VAC20-780-510-L
Description: Based on observation, the center failed to keep all medications locked.
Evidence:
1. In the office in a unlocked upper cabinet there was a bottle of Robitussin and children's Tylenol. The office door was not locked.

Plan of Correction: The medications will be picked up by parents or thrown away.

Standard #: 8VAC20-780-520-C
Description: Based on observation, the center failed to have diapering cream inaccessible to children.
Evidence:
1. Off the hallway there was a diapering table that had a diaper cream and an ointment that were stored on the bottom shelf of the diaper table.

Plan of Correction: The products will be removed today.

Standard #: 8VAC20-780-550-D
Description: REPEAT VIOLATION. Based on record review and interview, the center failed to complete all the required evacuation drills.
Evidence:
1. The months of Aug., Sept., Nov. Dec, and Jan. were not completed. 2. The director verified that the drills had not been completed.

Plan of Correction: An evacuation drill will be completed today. The inspector gave the director a model from to document drills.
