Technical Assistance:
Technical Assistance given and licensing standards regarding required postings were discussed and reviewed.

Comments:
An unannounced renewal inspection was conducted on March 11, 2024, from 5:30pm to approximately 6:30pm. There were 14 children present and two staff members. A sample size of eight records were reviewed which included five children?s records and three staff records. The Inspector observed staff and children?s interactions throughout the inspection. Children were observed during homework and free play. The majority of the children were being dismissed to their parents. First aid kits were observed. There are no medications administered at the center at this time. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented. The violations are listed on the violation notice issued to the center and were reviewed with the both staff member?s present at the exit interview.

Violations:
Standard #: 8VAC20-780-140-A
Description: REPEAT VIOLATION
Based on record review and interview it was determined that the center did not ensure that each child shall have a physical examination by or under the direction of a physician before the child's attendance or within 30 days after the first day of attendance.
Evidence:
1. Four of the five children?s records reviewed did not have documentation of a physical examination in their record.
2. Staff #1 confirmed that Child #1, Child #2, Child #3, and Child #4 did not have a physical examination documentation in their records.

Plan of Correction: The center responded with the following: Staff #1 will request a copy of the most recent physical examinations from the parents and will ensure that the physicals are in the children?s record.

Standard #: 8VAC20-780-160-C
Description: Based on record review and interview, it was determined that the center did not ensure that at least every two years from the date of the initial screening or testing, staff members shall obtain and submit the results of a follow-up tuberculosis screening.
Evidence:
1. Staff #1 and Staff #2 both have their tuberculosis tests last dated December 20, 2021, which is expired.
2. Staff #2 confirmed that both tuberculosis tests expired on December 20, 2023.

Plan of Correction: The center responded with the following: Staff#1 and Staff #2 will complete an updated tuberculosis screening this week and place the results in their case records.
