Technical Assistance:
Technical Assistance provided and licensing standards regarding required postings, children?s records, and menus were reviewed.

Comments:
An unannounced monitoring inspection was conducted on March 7, 2024, from 9:20am to approximately 12:30pm. There were currently 12 children present at the daycare with 3 staff members. The Inspector reviewed five children?s records and three staff records. The Inspector observed the interaction of staff and children throughout the inspection. Children were observed during group activity, free play, snack, bottle feeding and diapering. There are no medications administered at this time. Transportation is provided to and from school. Information gathered during the inspection determined non-compliances with applicable standards or law and violation was documented on the violation notice issued to the program. The violation was discussed with the owner/director at the exit interview.

Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observation and interview, it was determined that the center did not ensure that areas and equipment of the center, inside and outside, shall be maintained in a safe and operable condition. Evidence:
1. On the playground there are 4 large screws protruding from the playset apparatus which poses as an injury and tripping hazards for the children in care.
2. The baseboards of the playground area are coming off the gate which poses as a tripping hazard for the children in care.
3. Staff #1 confirmed that the screws and baseboards on the playground pose as injury and tripping hazards to the children.

Plan of Correction: The center responded with the following: The playset apparatus will be moved to storage until it is able to be fixed. The baseboards will either be removed or screwed back onto the gate depending on what is best determined by the repair staff.

Standard #: 8VAC20-780-500-B
Description: Based on observation and interview, it was determined that the center did not ensure that disposable diapers be disposed in a leakproof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal.
Evidence:
1. In both the infant and toddler rooms, the diaper disposal storage systems have broken lids. Staff members were observed touching the exterior surface of the storage system during disposal.
2. Staff #1, Director, confirmed that both diaper disposal storage systems are broken.

Plan of Correction: The center responded with the following: The center ordered two new garbage cans that evening.

Standard #: 8VAC20-780-560-G
Description: Based on observation and interview, it was determined that the center did not ensure that when food is brought from home, the food container shall be clearly dated and labeled in a way that identifies the owner.
Evidence:
1. The lunch boxes for all the children were not clearly dated and labeled in a way that identifies the owner.
2. Staff #1, Director, confirmed that the lunch boxes were not clearly dated and labeled in a way that identifies the owner.

Plan of Correction: The center responded with the following: The staff will have all lunch boxes dated and labeled.

Standard #: 8VAC20-780-580-B
Description: Based on observation and interview it was determined that the center did not ensure that any vehicle used by the center for the transportation of children shall meet the safety standards set by the Department of Motor Vehicles and shall be kept in satisfactory condition to assure the safety of children.
Evidence:
1. The booster seat used in the transportation vehicle expired in 2021.
2. Staff #1 confirmed that the booster seat is expired

Plan of Correction: The center responded with the following: The center purchased a new booster seat for the transportation van.
