Comments:
An unannounced monitoring inspection was conducted on 3/12/24 from 9:40am to 10:45am with the Director. There were seven children in care, ranging in age from two to five-years-old, supervised by one staff and one parent volunteer. The children were observed having free play in centers and getting ready to use the restroom and wash hands to go outside to the playground for snack. Four child records and two staff records were reviewed. The Center has two staff with current certification in CPR and First Aid, as well as trained in Daily Health Observation. The Director reported that there were no medications on site for children, but the program has staff who are current in Medication Administration Training (MAT). Required postings were observed. The attendance, menu, and emergency drill log were reviewed. The most recent Fire Inspection on file was dated 9/12/23 and the most recent Health Inspection on file was dated 9/21/23. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 3/21/24. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Standard #: 8VAC20-770-60-C-2
Description: Based on review of two staff records, the program did not obtain the results of a search of the central registry for one staff within 30 days of employment. Evidence: There were no central registry check results on record for Staff #2, with an employment date of 8/1/23.

Plan of Correction: Form has been resubmitted on 3/20/24

Standard #: 8VAC20-780-130-A
Description: Based on review of four child records, the program did not
obtain documentation that each child had received the immunizations required by the State Board of Health before the child attended the center. Evidence: The record for Child B, with a start date of 2/12/24, did not contain documentation of immunizations.

Plan of Correction: Parent provided immunization records on 3/18/24.

Standard #: 8VAC20-780-140-A
Description: Based on review of four child records, the program did not obtain a physical examination by or under the direction of a physician for each child within 30 days after the first day of attendance. Evidence: The records for Child B, with a start date of 2/12/24, and Child C, with a start date of 2/2/24, did not contain physicals.

Plan of Correction: Parents have been notified and we are expecting forms within the next week.

Standard #: 8VAC20-780-60-A
Description: Based on review of four child records, the program did not obtain all of the required documentation for child records. Evidence: 1. The records for Child B and Child D did not contain documentation of a work phone number and place of employment of each parent who has custody. 2. The records for B, Child C, and Child D, did not contain the address of two designated people to contact in an emergency if a parent cannot be reached. 3. The record for Child B did not contain documentation of any allergies and intolerance to medication or any other substances, and actions to take in an emergency situation.

Plan of Correction: Parents have been notified and files updated with the information.

Standard #: 8VAC20-780-240-A
Description: Based on review of two staff records, the program did not ensure that all staff completed the Virginia Department of Education-sponsored orientation course (Preservice) within 90 calendar days of employment. Evidence: The record for Staff #2, with an employment date of 8/1/23, did not contain documentation of completing the required Preservice training.

Plan of Correction: Training has been shared with staff and is being completed.

Standard #: 8VAC20-780-550-D
Description: Based on review of documentation and interview, the center did not ensure that a monthly practice evacuation drill was implemented. Evidence: The drill log did not contain documentation of a drill conducted in December 2023 or February 2024.

Plan of Correction: We have conducted 2 drills since the visit and are back on track.
