Technical Assistance:
Discussion of current staffing and enrollment.

A report from a monitoring inspection that was conducted on 09/14/2023 was signed by Staff #1. In addition, the file review conducted on 10/19/2023 was also discussed in order to determine the progress for each plan of correction.

Comments:
This monitoring inspection was initiated by licensing staff and concluded on 03/13/2024, between the hours of 3:30 pm to 5 pm. There were 24 school-age children present with 2 staff supervising. The children were engaged in the following: outside time, games, art, snack (cinnamon toast crunch served with milk), homework time, bathroom breaks, hand washing, and other organized activities. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; medication; special care and emergencies; and nutrition. A total of 5 children files were reviewed. Staff files were reviewed on an earlier date through the YMCA administrative office on 10/19/2023.

Information gathered during the inspection determined that there were (2) non-compliances with the applicable standards. These violations were documented on a Violation Notice issued to the center.

Violations:
Standard #: 8VAC20-770-60-B
Description: Based on review, 2 of 5 children records indicated that not all of the required documentation was contained within each file.

Evidence -
1. On the date of inspection (03/13/2024), documentation of health information (immunizations page and physical - as required by 8VAC20-780-130, 8VAC20-780- 140, and 8VAC20-780-150) were not available for: Child #1, and Child #4.
2. This was confirmed on the date of inspection by the Staff #1.

Plan of Correction: We will reach out to these parents to obtain this documentation.

Standard #: 8VAC20-780-430-K
Description: Based on observation and interview, provision were not made for an individual place for each child's personal belongings.

Evidence -
1. On the date of inspection (03/13/2024), individual cubbies were absent for each child's belongings. This was a departure as compared to the past when they were provided.
2. Staff #2 stated (during an interview conducted on the date of inspection) that the cubbies have not been made available to the children in care since the school has moved the start of the program day into the cafeteria.

Plan of Correction: The cubbies will be moved and left in the cafeteria until our return to the gym after snack/homework time.
