8VAC20-790 Subsidy Program Vendor Requirements for Family Day Homes

Comments:
A subsidy health and safety inspection (SHSI) was conducted on November 13, 2023 at the provider's new location to review subsidy requirements. There were two children present with one caregiver supervising. A monitoring inspection was also conducted on this date. Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-790-210-B
Description: Based on review, provider did not ensure caregiver has a negative TB screening/test at least every two years.

Evidence: The provider record has documentation of a TB screening/test dated October 19, 2021 which is more than two years old.

Plan of Correction: Provider will have TB test/screening completed on Saturday.

Standard #: 8VAC20-790-250-F
Description: Based on review and interview, provider did not ensure the department's health and safety update course is completed annually.

Evidence: The provider record had documentation of completing the department's health and safety update training on January 12, 2022. Provider confirmed she has not completed the update course since January 2022 which is more than a year old.

Plan of Correction: Provider will complete the 2023 update training.

Standard #: 8VAC20-790-260-A
Description: Based on observation, provider did not ensure areas and equipment of the family day home are maintained in a clean, safe and operable condition.

Evidence: Two cords were observed coming from the flat screen television mounted on the wall down to the electrical outlets. The cords were within reach of children.

Plan of Correction: TV cords will be secured and out of reach of children.

Standard #: 8VAC20-790-270-A
Description: Based on observation, provider did not ensure potentially poisonous substances are stored in areas inaccessible to children.

Evidence: Air freshener labeled "caution" and "Keep out of reach of children" was stored on the table in the childcare room within reach of children.

Plan of Correction: Air freshener was placed on a high shelf during inspection.
