Comments:
An unannounced complaint inspection was conducted on site on 3/1/2024 and an announced inspection with administrative staff at the admin building on 3/8/2024; in response to a complaint that was received by the Virginia Department of Education, Office of Child Care Health and Safety. Interviews were conducted with staff relating to allegation of Supervision, Forbidden Actions, Written Injury Reports. Based upon a preponderance of evidence, the information gathered during the investigation supports the allegation of Supervision, and lack of Written Injury Reports, and the complaint is determined to be valid. Please complete the columns for 'description of action to be taken' and 'date to be corrected' for each violation cited on the violation notice, and then return a signed and dated copy to the licensing office within 10 business days. Description: Based on staff interviews, the facility did not ensure that they have a qualified program director or a qualified back-up program director who meets one of the director qualifications who shall regularly be on site at least 50% of the center's hours of operation.
Evidence: identified through staff statements that the director of the program was Staff 1 and that they was not on site for at least 50% of the time. Staff stated that Staff 1 visited the site for meetings, but was not there 50% of the centers hours of operation. The facility has staff that may be director qualified, but they are considered lead teachers, and according to administration are classified as lead teachers, and not as program directors.

Description: Based on staff interviews, the facility did not ensure that while supervising children, they shall always ensure their care, protection, and guidance.
Evidence: identified through staff interviews, that a child was on the playground with their class and had asked to use the bathroom. The staff told the child to wait because they did not want to bring in a large group of children, or the whole group of children for one child. The child ended up relieving themselves on the playground and soiling their clothes. When staff were asked about the incident they stated that the child relieved themselves intentionally and not because they had to use the bathroom.

Description: Based on staff interviews and review of injury reports, the facility did not ensure that they notified parents by the end of the day of any known minor injuries.
Evidence: interviewed staff about injury and accident reports, and was told that they did not always document injuries when they gave a child an ice pack, band aid, or if the injury was just a small red mark on their skin. They also recalled an incident where two children ran into each other, hitting their heads, but no incident was documented.
