Comments:
An unannounced complaint inspection was conducted on 03/13/2024 in response to allegations received by the licensing office on 03/12/2024 in the areas of Staffing and Supervision, Physical Plant, Programs and Special Care Provisions and Emergencies. There were 23 children, ages 2 years - 5 years, and 4 staff providing direct care and supervision to children present during the inspection. Staff were interviewed and a total of 2 staff records were reviewed. The inspector arrived for the inspection at approximately 11:40 am and departed approximately 12:40 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

The information gathered pursuant to an investigation of the complaint supports the allegation and the complaint was determined to be valid.

Description: Based on record review, the center failed to ensure that each staff member shall submit documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with children.

Evidence: Staff 1 had a documented hire date of 06/14/2023. The tuberculosis screening was dated 06/15/2023. The screening was not submitted at the time of hire.

Description: REPEAT VIOLATION

Based on observation and discussion with staff, the center failed to ensure that for each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

Evidence:
1. Staff 3 confirmed there was no attendance record for 03/13/2024 in the Toddler 2 Room available for review. There were 11 children in care at the time the attendance record was reviewed. The attendance record did not document the arrival as it occurred.
2. In the Preschool Room, the attendance record for 03/11/2024 indicated 12 children in care and for 03/12/2024 the attendance record indicated 11 children in care. The attendance record did not document departure as it occurred.
3. In the Preschool Room, the attendance record for 03/13/2024 indicate 11 children in care. There were 12 children in care at the time the attendance record was reviewed. The attendance did not document the arrival as it occurred.

Plan of Correction: Staff will obtain addition training on proper attendance record keeping.

Description: COMPLAINT RELATED
Based on observation, the center failed to ensure that cribs, cots, mats, and beds used by children other that infants during the designated rest period or during evening and overnight care shall have linens consisting of a top cover and a bottom cover or a one-piece covering which is open on three edges.

Evidence:
1. In the Toddler 2 Room, during the designated rest period, there were 4 children on cots with only top cover.
2. In the Preschool Room, during the designated rest period, there were 6 children on cots with only top cover and 1 child with no top or bottom cover.

Plan of Correction: Staff will remind parents of the need for top and bottom cover and keep extras on site.
