Comments:
An unannounced monitoring inspection was initiated and completed on 03/8/2024. The inspector was on site from 7:30am until 11:00am.

There were 59 children, ages 9 weeks to 5years old under the supervision of 16 staff. Children were observed during indoor free play and breakfast time. Five children's records and five staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-160-A
Description: Based on record review, the center failed to have documentation of the TB screening submitted at the time of employment and prior to coming into contact with children.

Evidence:
1.Staff #5, which was hired on 10/5/2023 had a TB screening on file dating 10/9/2023 during the inspection on 3/8/2024.

Plan of Correction: The HR department will ensure that the staff has TB screening submitted at the time of employment.

Standard #: 8VAC20-780-80-A
Description: Based on interview, the center failed to ensure for each group of children, the center shall maintain a written record of daily attendance that documented the arrival and departure of each child in care as it occurs.

Evidence:
None of the classrooms had a written record of attendance in their classrooms.

Plan of Correction: We use tadpoles to check children in and out and will have the program director to ensure that they also have written record of daily attendance.

Standard #: 8VAC20-780-330-E
Description: Based on observation, the center failed to
ensure that sandboxes with bottoms which
precent drainage shall be covered when not in use.

Evidence:
The sandbox on the playground did not have a cover.

Plan of Correction: Maintenance will purchase a tarp to cover the sand box.

Standard #: 8VAC20-780-440-G
Description: Based on observation, the center failed to ensure that cribs shall be used for children under 12 months of age.
Evidence:
In the infant class staff were asked about children sleeping in cribs. Staff replied that some children sleep in cribs but others sleep in infant chairs due to not liking the cribs

Plan of Correction: The Program Director will ensure that all children under 12 months of age are sleeping in a crib.

Standard #: 8VAC20-780-500-A
Description: Based on observation, the center failed to ensure staff and children?s hands were washed with soap and running water after toileting.

Evidence:
1.Staff #6 was observed during the diapering process. There was no handwashing for child or staff after the diaper change.
2. Staff #7 was observed during the diapering process. There was no handwashing for child or staff after the diaper change.

Plan of Correction: The Program Director will provide training with teachers on handwashing for staff and children during toileting.

Standard #: 8VAC20-780-500-B
Description: Based on observation, the center failed to ensure that the diapering surface shall be cleaned with soap and at least room temperature water and sanitized after each use.

Evidence:
Multiples classrooms had no soap water for cleaning of diaper changing tables. When asked what they used for cleaning, Inspector was told they use the sanitizer or the disinfectant wipes.

Plan of Correction: The Program Director will ensure that the classrooms that diapering surfaces will be cleaned with soap and water and sanitized after each use.
