An unannounced renewal inspection was conducted on 3/18/2024 from 9:00am to 11:45am. At the time of entrance, 78 children were in care with 9 staff members present. Children were observed playing with puzzles, coloring, playing in centers, playing with blocks and legos and playing outside. (6) staff records and (6) children?s records, allergy action plans/medications, the physical space, evacuation drills and attendance records were reviewed. The site was clean and organized. Interactions between the children and staff were positive. Walls and bulletin boards were decorated with artwork. Areas of non-compliance are identified in the violation notice. Repeat violations were found and are identified within this report.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use staff names; list staff by positions only.

Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website.

Description: Based on record review, the center did not obtain a central registry report within 30 days
of employment for each staff member.
1. Staff #1 (Date of hire 10/16/2023) had central registry results dated 12/06/2023 and were not received within 30 days of employment. The center had followed up on 11/28/2023.
2.Staff #2 (Date of hire 12/12/2023) had central registry results dated 1/22/2024 and were not
received within 30 days of employment.

Plan of Correction: Management has reviewed and corrected our onboarding process to ensure all staff documents are obtained in the proper time frame. Management will follow up with the agency within 4 working days if not recieved in 30 days or request a new CRS within 5 working days after the contact

Description: Repeat Violation
Based on observation, hazardous substances were not kept in a unlocked place using a safe
locking method that prevent access by children.
1. In the Preschool 1 classroom, 5 cleaning bottles were inside an unlocked closet.

Plan of Correction: Management have met with and completed the Hazard Communication Program training with all staff members. Classroom cleaning guides have been reviewed and reposted. Staff have acknowledged that all chemicals must be locked and inaccessible to children at all times. All cabinet locks have been checked to
ensure they are in good shape and working properly.
