An unannounced monitoring inspection was conducted on March 18, 2024 from approximately 3:35 pm to 4:30 pm. Two staff were present supervising twenty-three school age children. A third staff arrived during inspection. Children were observed arriving to the program, washing hands, eating a afternoon snack and participating in various table activities. Five child and two staff records were reviewed.
Violation was found that is noted on the violation notice.

Standard #: 8VAC20-780-60-A Description: Based on review, facility did not ensure 1 of 5 child records has documentation of required information.
Evidence: The following was found during record review:
1. The record for child #1 (start date 1/22/2024) did not have a physical examination.
2. The record for child #2 (start date 9/5/2023) did not have a second emergency contact other than parent.Plan of Correction: Information will be requested from parents and added to child's record.
