An unannounced complaint inspection was initiated on 03/07/2024 at 11:00 a.m. and was concluded on 03/19/2024 at 12:30 p.m. The complaint was received on 2/29/2024 regarding staffing and supervision. Three classrooms were observed. Two staff records were reviewed.
Information gathered during the inspection supports at least one of the allegations and the complaint was determined to be valid. One complaint related violation was documented on the violation notice issued to the program.

Standard #: 8VAC20-780-350-B-2 Complaint related: Yes Description: Based on interview and document review, the center failed to maintain required staff-to-child ratios.
Evidence:
1. The center's request for the temporary staff-to-child ratio/group size waiver was approved requiring a staff-to-child ratio of 1:6 for children 16 months to 24 months.
2. On 03/07/2024, there were only two staff supervising 13 children. Staff #1 confirmed that the youngest child in the group was 23 months old and that the group was not within the required staff-to-child ratio of 1:6.Plan of Correction: 1. Frequent Headcounts**: Staff will conduct regular headcounts throughout the day to ensure compliance with the required staff-to-child ratio.
2. Age Awareness*: Staff will maintain an up-to-date list of children's ages to quickly identify any changes that may impact ratio requirements.
3. **Immediate Notification**: If a staff member realizes that the ratio is not being met, they will immediately notify a supervisor.
4. **Supervisory Action**: Supervisors will take prompt action to ensure that the appropriate staff-to-child ratio is restored, either by reassigning staff or temporarily reducing group sizes.
5. **Training**: All staff was re trained on the importance of maintaining proper staff-to-child ratios and the procedures for monitoring and reporting.
