An unannounced renewal inspection was conducted on 3/14/24 from 11:50am to 2:30pm. At the time of the inspection there were 57 children in care and 11 staff members. Children were observed participating in outdoor play, educational and art activities. Records were reviewed for five children and five staff members. Evacuation drills, emergency supplies and other required records and postings were also reviewed. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice and discussed during the exit interview.

Standard #: 8VAC20-780-60-A-8 Description: Based on review of children?s records and interviews, it was determined that the center did not ensure there was a written care plan for each child with a diagnosed food allergy that included instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
Evidence:
1.The record for child #4 contained documentation of a diagnosed food allergy to soy and dairy. The record did not contain a written care plan with instructions from a physician.
2. The Director confirmed there was no documentation of a written care plan.Plan of Correction: The attendance forms will be updated to include arrival and departure times.
Standard #: 8VAC20-780-80-A Description: Based on record review and interviews, it was determined the facility did not ensure that a written daily record of children?s daily attendance, including arrival and departure times, was maintained.
Evidence:
1.The attendance maintained by the center did not include documentation of children?s arrival and departure times.

Standard #: 8VAC20-780-550-P Description: Based on observation and interviews, it was determined that the facility did not ensure that the center maintain a written record containing all the required information of children's serious and minor injuries.
Evidence:
1.The record for child #6 did not include the time the parent was notified of an injury.
2.The Director confirmed the time of notification was not documented.Plan of Correction: Staff will document the time a parent was notified of an injury.
