An unannounced focused inspection was conducted on 03/18/2024 from 10:40 a.m. to 10:55 p.m. The inspector reviewed compliance in the area of physical plant.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-330-B Description: REPEAT VIOLATION
Based on measurement and interview, the center failed to ensure that where playground equipment is provided, resilient surfacing complies with the minimum safety standards.
Evidence:
1. The pea gravel beside of the blue climber measured four inches. And in front of the blue climber, the pea gravel measured one inch.
2. The pea gravel did not meet the minimum safety standard of six inches.Plan of Correction: SEE INTENSIVE PLAN OF CORRECTION
