Comments:
An unannounced monitoring inspection was conducted on 3/18/2024. There were 29 children, ages 5 -10 years and 2 staff present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, care of children, preventing the spread of disease, emergencies and nutrition. A total of 3 children?s records and 1 staff record were reviewed. The children were observed during arrival, handwashing and snack time.
The inspector arrived for the inspection at 2:27pm and departed the center at 3:43pm.

Discussion on standards related to operational responsibilities.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Violations:
Standard #: 8VAC20-770-60-C-2
Description: Based on the review of one staff record, the center failed to ensure that central registry results were obtained by the end of the 30th day of employment.

Evidence: Staff #1 start date was 9/7/2023 and on the day of the inspection the central registry results were not obtained.

Plan of Correction: Provider will obtain central registry results within 30 days of hire.

Standard #: 8VAC20-780-70
Description: Based on review of one staff record, the center failed to ensure that all records were complete.

Evidence: Staff #1 start date was 9/7/2023 and on the day of the inspection did not have: documentation to demonstrate education; Information about any health problems that may interfere with fulfilling the job responsibilities.

Plan of Correction: Provider will obtain documentation of education and information on health problems.

Standard #: 8VAC20-780-240-B
Description: Based on the review of one staff record, the center failed to ensure that staff shall complete orientation training prior to the staff member working alone with children.

Evidence: Staff #1 start date was 9/7/2023 and on the day of the inspection did not have orientation training prior to working with children.

Plan of Correction: Provider will ensure orientation training is completed.
