Comments:
An unannounced renewal inspection was conducted on January 30, 2024. The inspector was on site from approximately 9:50 am until 12:15 pm. There were 22 children present with seven staff supervising. Pre-k room was observed playing with magnetic tiles and blocks, learning new skills with numbers, and sharing toys. Kids engaged in play with inspector, showing their favorite toys and building blocks.

The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records, six staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word corrected is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Violations:
Standard #: 8VAC20-780-80-B-2
Description: Based on a report submitted by the center and interview, the center failed to inform the department's representative as soon as practicable, but not to exceed two business days, of any injury to a child that occurs while the child is under the supervision of the center and requires outside medical attention.

Evidence:
A report of injury of Child A (1 year of age) Requiring Outside Medical Treatment was dated 1/22/2024. The provider documented and submitted the injury report to the department dated 1/30/2024.

Plan of Correction: The Director will report all injuries that require outside treatment to DOE within 24 hours of occurrence.

Standard #: 8VAC20-780-245-I
Description: Based on observation and interview, it was determined that the center did not ensure that the emergency preparedness plan was reviewed annually.

Evidence:
1.T he emergency plan?s last update was listed as 6/21/22.
2. Staff #1 confirmed that 6/21/22 was the last time it was updated.

Plan of Correction: The Director will make sure that the emergency plans are viewed and updated every year.

Standard #: 8VAC20-780-270-A
Description: Based upon observation and interview, the facility has not ensured that areas and equipment, inside and outside, are maintained to be clean and safe.

Evidence:
1. The post of the fence that surrounds the playground area contained areas of splintered wood, that could entangle clothing, snag skin or cause splinters.
2. Staff #1 confirmed the post had sharp edges.

Plan of Correction: The Director is aware of the splintered wood on the posts and is looking into another options of fencing as well as getting this fixed,

Standard #: 8VAC20-780-440-B
Description: Based on observation and interview, it was determined that the center did not ensure that cribs be identified for use by a specific child.

Evidence:
1. In the infant room with five children present, a total of five cribs did not have labels or names that identified each crib use by a specific child.
2. Staff #1 confirmed that the cribs were not labeled with the children?s names.

Plan of Correction: The Director will make sure that all cribs are marked with baby's names.

Names will be changed when new children join us, and the infants change
classes .
