The inspection findings have been updated as a result of a first step review completed on 03/28/2024.
The licensing inspector initiated an unannounced inspection on 03/12/2024 and concluded it on 03/13/2024 in response to a complaint received by the department on 03/07/2024 for allegations regarding physical plant and care and protection. Interviews, record reviews and policy reviews were conducted relating to the allegations. The information gathered during the investigation support the allegation, so the complaint is determined to be valid, and violations were documented on the violation notice issued to the facility.

Evidence:
On 03/06/2024 Child 1, three years old, was standing on a ladder, measuring 74 inches and located over a bare floor and without resilient surfacing, attempting to return a toy to a shelf located 78 ? inches above the floor when the child fell to the ground, resulting in a left temporal bone fracture with pneumocephalus and a hematoma behind the child's left ear.Plan of Correction: Not available online.
