Comments:
An unannounced monitoring inspection was conducted on March 21, 2024 between the hours of 10:15am-11:30am. There were a total of 61 children with 11 supervising staff members present. Children were observed during snack time, handwashing, centers,engaged in group activities and outdoor play time. A selection of staff and children records, medications and other documents were reviewed. The physical space, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Standard #: 8VAC20-780-60-A-8
Description: Based on record review, the center did not obtain a written care plan for each child with a diagnosed food allergy, to include instructions from the physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
Evidence:
1. Child #6 has emergency medication on site and is diagnosed with a food allergy. Child #6's record did not contain a written care plan for a diagnosed food allergy.

Plan of Correction: The administrative team will give the FARE form to the parents to have the physician to complete the form.
