Comments:
An unannounced monitoring inspection was conducted from 11:15 am - 2:15pm with the center director. There were 25 children, ranging from eight months to five years of age, present with five staff supervising. Infant care was observed and older children were observed eating lunch, reading books, and preparing for nap. Four child records and three staff records were reviewed, along with the emergency drill log, written daily attendance, written allergy list, six medications and authorization forms, and the required posted information.

Violations:
Standard #: 8VAC20-780-270-A
Description: (REPEAT VIOLATION) Based on observation, the facility failed to ensure that all areas inside and outside are maintained in a safe condition. Evidence:
1) The playhouse has a cracked roof, which is sitting inside the playhouse
2) There is a broken tile in the bathroom which is a tripping hazard
3) There is a broken purple car with cracks and sharp plastic exposed
4) There is deteriorating wood on the picnic benches/tables, and wooden equipment
5) There are multiple pieces of wood along the bottom of the fence with protruding nails/screws
6) There is a broken glass door leading to the four?s classroom from the playground
7) There are multiple tree roots that are exposed causing a tripping hazard
8) There are multiple fence pickets that are cracked/broken; one fence picket is missing
9) In the infant classroom, there were four cribs with loose fitting sheets. Loose crib sheets are a suffocation hazard and increase the risk of sudden unexplained infant death.

Standard #: 8VAC20-780-330-B
Description: (REPEAT VIOLATION) Based on observation, there was not resilient surfacing around playground equipment, and a fall zone, as required. Evidence:
1) the resilient surfacing measured behind the swings was approximately 9 feet, and the fence is approximately 13 feet 6 inches from the center of the swing. The height of the swing beam is approximately 7 feet high which requires a fall zone with resilient surfacing of 14 feet (measured from the swing itself) in both the front and back of the swings. The ?pea gravel? looked like river rocks and is not an approved material on the compressed loose fill synthetic materials depth chart.
2) There is less than 2? of resilient surfacing under and around the composite play equipment.
3) The bicycle play equipment (bikes on track) need a fall zone with 6 feet of resilient surfacing on all sides
4) There is not any resilient surfacing under and around the slide on the small playground.

Description: Based on observation, the facility failed to ensure that staff wash their hands, as required. Evidence: In the two's classroom, staff didn?t wash their hands before and after each diaper change.

Description: Based on observation, and staff interview, the facility failed to ensure that all diapering procedures were followed, as required. Evidence: In the two's classroom, staff did not sanitize the diapering surface, as required, after each use.

Description: Based on review of documentation and staff interview, it was determined that the facility failed to follow their own medication procedures as required. Long term prescription drug use and over-the-counter medication may be allowed with written authorization from the child?s physician and parent. Evidence: the records for child #5 and child #6 did not contain complete written authorization from the child's parent and physician for long term emergency medication that was observed on site.

Description: Based on observation, and staff interview, the facility failed to follow USDA nutritional requirements when serving lunch. Evidence: the children were observed eating turkey kielbasa, rice, and pineapple for lunch; no vegetable was served.
