Comments:
An unannounced renewal inspection was conducted on 03/21/2024. A tour of the facility was conducted, in addition to a record review and interviews with staff. The Inspector reviewed 3children?s records and 2 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-260-A
Description: Based on a record review the center did not have documentation of having an annual fire inspection.
Evidence:
The last documented fire inspection report was dated 06/06/2022.

Plan of Correction: The Fire Marshal will be contacted to schedule a fire inspection.,

Standard #: 8VAC20-780-270-A
Description: Based on observation the center did not ensure all areas and equipment of the center were maintained in a safe condition.
Evidence:
1. There was a broken floor tile in the classroom.
2. The play stove handles had one with a sharp screw exposed in reach of children.
3. There was a tall cubby that was not secured in any way to prevent tipping.

Plan of Correction: The cubby will be secured to the wall and the screw sticking out of the play stove handle will be covered to prevent injury to children. Maintenance will be notified about the broken tile.

Standard #: 8VAC20-780-290-A-3
Description: Based on observation the center did not ensure all electrical outlets had protective covers.
Evidence:
There was an electrical outlet near the reading are, that did not have a cover.

Plan of Correction: Staff will check all outlets in the morning to ensure that they are covered.
