An unannounced monitoring inspection was conducted on March 22, 2024 between the hours of 9:00am-11:00am. There were a total of 84 children with 15 supervising staff members present. Children were observed during bottle feeding, handwashing, circle time, engaged in group activities and outdoor play time. A selection of staff and children records, medications and other documents were reviewed. The physical space, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. Repeat Violations were cited. Standard #: 8VAC20-770-60-C-2 Description: Based on record review and interview, the center did not obtain a central registry finding within 30 days of employment.
Evidence:
1. Staff #4's record (Date of hire-11/06/2023) did not contain a central registry finding within 30 days of employment. The central registry finding was dated 03/01/2024.
2. Staff #6 confirmed that Staff #4's central registry finding was not within 30 days of the date of hire and was an oversight.Plan of Correction: Management will submit a search for a central registry result within 7 days of employment and follow up with the office of background of investigations if results are not returned in 30 days.
Standard #: 8VAC20-780-70 Description: Repeat Violation
Based on record review and interview, required records were not kept for each staff.
Evidence:
1.Staff #5's record (Date of hire-03/19/2024) did not contain documentation of two or more references as to character and reputation as well as competency before employment.
2. Staff #6 confirmed that Staff #5's references were not checked prior to the date of hire.Plan of Correction: References will be checked today. Management will ensure that all references are checked before the date of hire.

Based on observation, areas and equipment of the center, inside and outside, were not maintained in a clean, safe and operable condition.
Evidence:
1. One of three toilets located in Classroom #4 did not flush. The toilet bowl contained urine.Plan of Correction: A work order will be placed to day to get the toilet fixed.
Standard #: 8VAC20-780-520-C Description: Based on documentation review, written parent authorization for diaper ointment was not obtained by the center.
Evidence:
1. Child #7 did not have written parent authorization for a diaper ointment located in the classroom.Plan of Correction: The parents will complete a written authorization for the diaper cream today.
