Comments:
A non-mandated monitoring was conducted on 3/13/2024 in response to an injury report requiring outside medical treatment received by the office on 3/7/24. On 3/5/2024 a child, age 4, was reported to have fallen over the slide and landed on the ground hurting their arm. A parent later reported the injury was a break in the left arm. The inspection was concluded on 3/22/2024.

The inspector was on site from 1:40pm until 2:45pm. One children's records and two staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-330-B
Description: Based on observation, the center failed to ensure that all climbable playground equipment had the required resilient surfacing. A fall zone shall encompass sufficient area to include the child's trajectory in the event of a fall while the equipment is in use.

Evidence:
On 3/13/2024, observed a non-measurable amount of resilient surfacing under the required fall zone in front of the larger slide of the climbable playset. On the side of the slide, there was approximately 3 inches of resilient surfacing. The required amount is 6 inches of resilient surfacing.

Plan of Correction: Mulch was added on 3/14/2024.
