Comments:

An unannounced monitoring inspection was conducted on 3./22/2024 between the hours of 12:00pm-1:30pm. 29 children were observed directly supervised by 5 staff. The physical plant, 3 staff records, 3 children?s records, medications, and evacuation drills were inspected. Children were observed having snack and preparing for naptime. Areas of non-compliance are identified in the Violation Notice. Standard #:	22.1-289.035-A
Description:	Based on record review, the center did not obtain a fingerprint-based criminal history record check every five years for 1 of 3 staff reviewed. Evidence: 1. The fingerprint based criminal history record check results in the record for Staff #3 were dated 08/10/2023. The preceding fingerprint check results in the record were dated 06/14/2018, more than 5 years prior to the date of the most recent results.
Plan of Correction:	The center will make sure all repeat background checks are obtained every 5 years.

Description:	Based on record review, 2 of 3 children's records did not contain all required information. Evidence: 1. The record for Child #1 did not contain the telephone number for the place of employment for one parent. 2. The record for Child #2 did not contain the telephone number for the place of employment for each parent.
Plan of Correction:	The center will request the information from the parents.
Standard #:	8VAC20-780-240-A
Description:	Based on record review, 1 of 3 staff reviewed did not complete the Virginia Department of Education-sponsored orientation course within 90 calendar days of employment. Evidence: 1. Staff #2 began employment on 9/20/2023. 2. The record for Staff #2 did not contain documentation of the completion of Virginia Department of Education-sponsored orientation course
Plan of Correction:	Staff #2 will be required to complete the preservice training by 4/1/2024.
Standard #:	8VAC20-780-240-E
Description:	Based on record review, 1 of 3 staff did not complete orientation training in first aid and cardiopulmonary resuscitation (CPR) within 30 days of the first day of employment. Evidence: 1. Staff #2 began employment on 9/20/2023. 2. The documentation of first aid and CPR training in the record for Staff #2 was dated 11/17/2023, more than 30 days from the first day of employment.
Plan of Correction:	The center will ensure staff receive orientation training in first aid and CPR within 30 days of employment.

Description:	Based on record review, practice evacuation drills were not completed monthly. Evidence: 1. The emergency drill log did not contain documentation of an emergency evacuation drill for the months of January 2024 and February 2024.
