An unannounced mandated inspection was completed from 3:20 until 4:50pm by two inspectors. There were 11 children present. There were two staff present from 3:20 until approximately 4:00 when another staff person arrived. Four children?s records and three staff records were reviewed.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition.
Information gathered during the inspection determined non-compliance with applicable standards or law and violation (s) were documented on the violation notice issued to the program.

Based record review, the facility failed to ensure that all repeat fingerprints checks are conducted every five years.

Standard #: 22.1-289.035-B-2 Description: Based on record review, the center failed to ensure that no person shall be hired for compensated employment prior to the facility obtaining fingerprints check eligibility letters.

There was an emergency card available, but it did not contain home address, place of employment for each parent and phone #, name and phone # for child's physician, emergency addresses, chronic physical problems, first date of enrollment, previous child care attended, or concurrently attended, and a physical examination and immunization record.
