A subsidy health and safety (SHSI) inspection were initiated on 03/20/2024 and concluded on 03/20/2024. There were 13 children present with 4 staff supervising ranging in age from 2 to 4 years. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.
Information gathered during the inspection determined non-compliance with subsidy regulations and violations were documented on the violation notice issued to the facility.

Standard #: 8VAC20-790-600-F Description: Based on a review of records, the vendor did not ensure that each staff complete the Virginia Annual Health and Safety Update Training.
Evidence:
1. The record of Staff 1 (DOH 10/05/04) contained documentation of the 3-hour annual health and safety update training dated 08/05/2021.Plan of Correction: I will make sure that all staff complete the annual update yearly.
