Comments:
An unannounced renewal inspection was conducted on 3/25/2024 from 2:50 pm - 5:50 pm. At the time of entrance, 53 children, ages 4 months to 5 years old, were in care with 13 staff members present. The sample size consisted of 5 children?s records and 10 staff records. Children were observed engaged in circle time, playing in centers, and dancing to music. Infants were observed having an snack at the bucket table, being held in staff's lap and sitting on the carpet watching a children's program. Medication administration was not observed. The program director states that there is no medication being administered to children in care at this time. Transportation was not observed and the vehicle used for transporting children is not kept on site. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-770-60-C-2
Description: Based on a review of records, the licensee did not always ensure that staff's results of the central registry search are received within 30 days of hire - or communication with office of background checks (OBI) is documented regarding the delay of results and steps to be taken in order to obtain the results.
Evidence:
Ten staff records were reviewed.
The record for staff # 8, whose date of hire was documented as 5/17/2022, contained the results of a Search of the Central Registry which was dated 7/1/2022, more than 30 days after Staff #8's employment.
The program director was unable to demonstrate that communication with office of background checks (OBI) took place regarding he delay in results.

Plan of Correction: The program director states that background check requests are being sent out within 10 days of hire and requests are followed up on if results are not back before 30 days. The program director will ensure that documentation of communication / follow-up is kept in the staff records in case this documentation is needed in the future.

Standard #: 8VAC20-780-240-B
Description: Based on a review of records and interview, the licensee did not always ensure that staff complete orientation training within 7 days of employment.
Evidence:
Ten staff records were reviewed.
The record for staff #6, whose date of employment was listed as 6/27/2022, listed the date that orientation was completed as 7/11/2022.
Orientation was completed more than 7 days following staff #6's date of hire.
The program director stated that she did not have documentation showing otherwise.

Plan of Correction: The licensee is now ensuring that staff complete orientation within 7 days of hire.
This staff was employed in 2022 and the timing has been corrected since then.

Standard #: 8VAC20-780-245-A
Description: Based on a review of records and interview, the licensee did not ensure that all staff complete a minimum of 16 hours of training, appropriate to the age of children in care, annually.
Evidence:
10 staff records were reviewed.
The record for staff # 6, hired 6/27/2022, contained only 8 of the required 16 hours of annual training for 2023.
The record for staff #8, hired 5/17/2022, contained only 3 of the required 16 hours of annual training for 2023.
The record for staff #9, hired 6/28/2021, contained only 9 of the required 16 hours of annual training for 2023.
The program director did not have additional documentation to demonstrate that these staff completed a total of 16 hours of annual training last year (2023).

Plan of Correction: Staff had a training day just last week and these training hours, as well as any additional training hours for the year, will be documented on a log kept in each staff record in order to ensure training taken is counted toward the required annual 16 hours for each staff.
