Technical Assistance:
The inspector reviewed the following with the program director:
*suggested raking woodchips at the bottom of the slide to maintain sufficient resilient surfacing.
* discussed standard 8VAC20-780-290.3 (outlet covers)

Comments:
An unannounced renewal inspection was initiated on 03/27/2024 at 9:51 am and completed on 03/27/2024 at 1:10 pm.

At the time of entrance there were 90 preschool children in care with ten teaching staff. Children were observed during morning program time which included large group, free play and outdoor play. Lunch and nap was additionally reviewed.

Records were reviewed for seven staff and twelve children in care. Injury reports, medication and emergency supplies were also reviewed.

Areas of non-compliance are identified on the violation notice and were discussed with the director at the conclusion of the inspection.

Violations:
Standard #: 8VAC20-780-240-A
Description: Based on record review and interview, the center failed to provide documentation that staff have completed The Virginia Department of Education-sponsored orientation course shall be completed within 90 calendar days of employment.

Evidence:
1. Training certificates for staff 1 and staff 2 were not available to verify completion of Virginia Preservice Training.
a. Staff 1 and staff 2 have been employed more than 90 days.
2. Administrative staff confirmed that training certificates were not available to demonstrate that staff 1 and staff 2 had completed this training.

Plan of Correction: Administrative staff research is being conducted to determine if staff 1 and staff 2 have completed Virginia Preservice Training. If the training needs to be completed - both staff will complete.

Standard #: 8VAC20-780-245-A
Description: Based on document review and staff interview, the center failed to ensure that staff shall complete annually a minimum of 16 hours of training appropriate to the age of children in care.

Evidence:
1. The written training log provided lacked any documentation of training hours therefore, no verification could be made to determine if staff 2, staff 3, staff 4 and staff 5 obtained the required 16 hours of annual training in 2023.
2. Administrative staff confirmed that the training spread sheet provided lacked documentation of training hours and confirmed that additional documentation was not available at this time.

Plan of Correction: Administrative stated that records are being reviewed and updated to accurately include training hours for all staff. If staff lack 16 hours - additional training will be obtained.

Standard #: 8VAC20-780-270-A
Description: Repeat Violation
Based on observation the center failed to ensure that areas and equipment of the center, inside and outside, shall be maintained in a safe and operable condition.

Evidence:
1.The following safety hazards were observed on the outdoor play area:
a. The wooden shed has multiple areas of broken and splintered wood. Additionally, an area of wood with protruding nails was dangling down from the top of the shed and a metal bracket was hanging loose from the bottom area of the shed.
b. One metal bolt, that is used to secure the playground border, is protruding approximately two inches.
c. There were widespread areas of peeling paint on the wooden railing that is used for an entrance/exit from playground.
2.The following safety hazards were observed inside of the facility:
a. One of the mini blinds in classroom D has multiple areas that are broken and split apart.
b. There is an area of drywall in the bathroom of classroom D with sharp points, as multiple holes were not patched.

Plan of Correction: Administrative staff stated that a maintenance request would be submitted for all repairs.

Standard #: 8VAC20-780-280-B
Description: Repeat Violation
Based on observation and interview the center failed to ensure that hazardous substances such as cleaning materials be kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. A bottle of bleach and disinfectant cleanser was observed stored on the bottle shelf of an unlocked cabinet in Classroom E.
2. Staff stated that the cabinet lock no longer works.

Plan of Correction: Chemicals were locked up during the inspection. Staff stated that a maintenance request would be submitted to have the cabinet repaired.

Standard #: 8VAC20-780-550-B
Description: Repeat Violation
Based on record review and interview it was determined the center did not ensure the emergency preparedness plan contained procedural components for plan review.

Evidence:
1. The written emergency preparedness plan was last updated in January 2023.
2. The program director stated that this was the most recent plan on file at the facility.

Plan of Correction: Program director stated that she would reach out to HRCAP administrative staff to discuss an annual update/review of the center's emergency plan.
