Comments:
An unannounced monitoring inspection was conducted on 3/28/2024. The inspector was on site from approximately 9:25 am-11:20 am. There were 24 children present, ranging in ages from 18 months to 5 years, with 6 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies, nutrition and background checks. A total of 5 child records and 5 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word corrected is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Violations:
Standard #: 8VAC20-780-160-A
Description: Based on a review of five staff records and interview, the center did not ensure to obtain documentation of a negative tuberculosis (TB) screening for one staff at the time of employment and prior to coming into contact with children as required.

Evidence:
1. The record of staff #3 (date of employment: 3/25/2024) did not contain a TB screening.
2. Administration acknowledged that the TB screening had not been completed.

Plan of Correction: All paperwork will be collected from future employees before hire dates. Staff #3 has scheduled an appointment to complete TB tests with her doctor.

Standard #: 8VAC20-780-500-B
Description: Based on observation and interview, the center did not ensure that disposable diapers were disposed in a leakproof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal as required.

Evidence:
1. A foot operated trash can or comparable receptacle was not observed in the toddler classroom.
2. Administration acknowledged that they did not have the correct trash can for diapers in the toddler room.

Plan of Correction: A foot operated trash can was ordered and placed in the toddler classroom to dispose of diapers.
