Comments:
An unannounced monitoring inspection was conducted on 3/21/24 from 4:50pm to 5:50pm with program staff. There were 28 children in care at the start of the inspection, supervised by three staff. By the end of the inspection there were 15 children in care. Children were observed playing in centers. Required postings were observed. The attendance, menu, allergy list, and emergency drill log were reviewed. Four child records were reviewed. A review of required on-site documentation was conducted for staff certified in CPR/First Aid, Daily Health Observation Training, and Medication Administration Training (MAT). Off-site staff records were reviewed on 3/22/2024. Nine medications and authorizations were reviewed. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Violations:
Standard #: 8VAC20-770-60-C-2
Description: Based on review of staff records, the program did not obtain the results of a search of the central registry within 30 days of employment as required. Evidence: The central registry results on record for Staff #3, with an employment date of 7/1/23, were dated 8/15/23.

Plan of Correction: Manager will contact central registry before 30 days of employment and document.

Standard #: 8VAC20-780-60-A
Description: Based on review of four child records, the program did not obtain all of the required documentation for child records. Evidence: The record for Child C did not contain the address and phone number of a second person to contact in case of emergency if a parent cannot be reached.

Plan of Correction: Updated child's file with correct information.

Standard #: 8VAC20-780-80-A
Description: Based on observation, review of documentation, and interview, the center did not maintain a written record of daily attendance that documented the arrival and departure of each child in care as it occurred. Evidence: On 3/21/24 at approximately 4:50pm the Licensing Inspector (LI) arrived at the program and Staff #1 reported that the attendance was not current due to children just arriving from tutoring. At approximately 5:37pm Staff #1 provided the attendance to the LI and stated that it was updated. At that time the LI observed 15 children in the program. The attendance contained documentation of 12 children in care.

Plan of Correction: Counseled staff that all children must be checked in/out on roster.

Standard #: 8VAC20-780-240-A
Description: Based on review of staff records, the program did not ensure that the Virginia Department of Education-sponsored orientation course (Preservice) was completed within 90 calendar days of employment. Evidence: The Preservice certificate on record for Staff #3, with an employment date of 7/1/23, was dated 3/12/24.

Plan of Correction: Will insure all trainings will be completed within timeframe of hire.
