Comments:
An unannounced renewal inspection was conducted on 3/21/24 from 3:40pm to 4:30pm with program staff. There were 34 children in care, supervised by five staff. Children were observed playing in centers, playing a group game, and transitioning to wash hands for snack. Required postings were observed. The attendance, menu, allergy list, and emergency drill log were reviewed. Five child records were reviewed. A review of required on-site documentation was conducted for staff certified in CPR/First Aid, Daily Health Observation Training, and Medication Administration Training (MAT). Off-site staff records were reviewed on 3/22/2024. Four medications and authorizations were reviewed. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 4/9/24. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Standard #: 8VAC20-780-160-A
Description: Based on review of staff records, the program did not obtain the results of a tuberculosis (TB) test or screening prior to employment. Evidence: The TB test on record for Staff #1, with an employment date of 8/28/23, was dated 9/8/23. The TB test on record for Staff #2, with an employment date of 10/10/23, was dated 10/23/23.

Plan of Correction: Manager will make sure to follow timeline set forth by DOE.
