Technical Assistance:
Discussed the process of out of state background check requests. Recommended an audit of all staff files to ensure all background check clearances are on file.

Comments:
An unannounced Renewal inspection was conducted today. The Head of School was on leave and the Assistant Head of School was onsite. A total of 74 children with 14 staff were onsite today. A total of 7 classrooms were occupied, all 7 were observed. Some classes transitioned to the large auditorium for activities. The staff in the Nido class was assisting with spoon feeding infants, bottle feeding and providing tummy time for one infant. In some of the Children's House classes, staff were assisting the children with making holiday eggs which included gluing eyes and hands on the plastic egg shell. In class #5 green and yellow paintings were on display that were completed by the children. The group was sitting in a circle singing and listening to staff reading "The Little Cricket". Transitions from hand washing to lunch was also observed. All medication onsite was reviewed. A total of 6 children's records and 5 staff records were reviewed. Based on staff record review, background checks were not completed for one staff.
Evidence:
Staff #3 had out of state residency in two states. An out of state Central Registry clearance and an out of state Criminal Record clearance was not obtained.

Plan of Correction: A background check has been submitted in the portal, waiting for results.

Standard #: 8VAC20-780-60-A
Description: Based on child record review, two child records were incomplete.
Evidence:
1. Child #2 and #3's records were missing addresses for one of the two emergency contacts.
2. Child #3's record did not have a second emergency contact.

Plan of Correction: emergency contact addresses have been updated and confirmed

Description: Based on review of staff records, 2 staff did not receive all topics of required training.
Evidence:
The orientation training staff #2 and #3 received did not include training on SIDS/Shaken Baby/Head trauma and food allergies.

Description: Based on observation of the classrooms, one area was not maintained in a safe condition.
Evidence:
A gray chair in Class #3 had loose legs and one leg bent causing the chair to be unstable.

Plan of Correction: Furniture: the grey chair has been removed from classroom #3

Standard #: 8VAC20-780-280-B
Description: Based on observation of the classrooms, cleaning materials were unlocked and accessible to children.
Evidence:
Cleaning agents were found in an unlocked cabinet beneath the sink in Class #2. A cleaning agent was found on a shelf in the bathroom in Class #4.

Plan of Correction: Cleaning Supplies: a refresher to staff on how cleaning supplies should be stored away and locked even in the bathrooms

Standard #: 8VAC20-780-350-C
Description: Based on observation of twp ongoing mixed age groups, ratios did not apply to the youngest child in each group.
Evidence:
1. Class #13 had a total of 12 children with 1 staff.
2. The class was not a balanced mixed age group. There were 5 three year old's, 5 four years old's and only 1 five year old.
3. Class #2 had a total of 14 children with 1 staff when the Inspector entered the classroom.
4. The class was not a balanced mixed age group. There were 6 three year old's, 5 four year old's and 3 five year old's.
3. A ratio of 1 staff to 10 children was not maintained in either classroom.

Plan of Correction: Ratio: spoke with Classroom #13 and #2 on ratio and when they are balanced

Description: Based on observation of the center, required postings were not found.
Evidence:
A daily schedule was not posted for classrooms # 6 and #13.

Description: Based on review of the center's written record of children's injuries for January through March, the record was incomplete.
Evidence:
The following injury reports did not include the date and time the parent was notified:
01/05/24, 01/10/24, 01/23/24, 01/24/24, 01/26/24, 02/15/24, 02/23/24, 02/26/24,02/29/24 and 03/21/24.
