An announced initial inspection was conducted between 9:30 A.M.-11:20 A.M. on April 1, 2024. The licensing inspector conducted a tour of the physical plant, verified floor and site plans, and reviewed the background check findings for the applicant/agents and the program director. As no children were in care at the time of the inspection, the center representative was reminded that when one or more children are in care there shall always be one staff member who meets the qualifications of a program leader or program director and an immediately available staff member, volunteer or other employee who is at least 16 years of age with direct means for communication between the two of them. Policies and procedures submitted with the initial application were reviewed prior to the inspection. Areas of non-compliance are identified in the Violation Notice.
Please return a signed and dated copy of each page of this notice to the licensing inspector no later than 5:00 P.M. on April 10, 2024. If you?d like to include a plan or correction and date to be corrected for each violation cited, please include that information with the signed and dated copy of the Violation Notice by the deadline. Standard #: 22.1-289-036-B-4 Description: Based on record review, the center failed to obtain a copy of the results of a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.
Evidence:
1.Applicant #4 indicated on a sworn disclosure statement dated 02/28/2024 that they had lived outside of Virginia within the last five years.

Standard #: 8VAC20-770-40-C-1-a Description: Based on record review, the center failed to have required background checks at the time of initial application for licensure.
Evidence:

Standard #: 8VAC20-780-270-A Description: Based on observation, the center failed to ensure that all areas and equipment of the center shall be maintained in a clean, safe, and operable condition.
Evidence:
1. In the Jordan Classroom there were two holes in the wall next to the window.
2. In the Twyman Classroom there were two holes in the wall above the mirror as you entered the classroom, two holes to the left of the mirror, and peeling paint in several areas.
3. In the Ferguson Classroom there was peeling paint in several areas, a tile in front of the restroom that was broken and cracked, and a loose outlet plate cover over by the dramatic play area.
4. In the restroom off the Ferguson Classroom there were several missing pieces of tile in front of the toilet, the toilet paper holder was broken, there were areas of peeling paint, and two holes in the wall above the soap dispenser.
5. The floors throughout the Ferguson Classroom dirt was caked in the corner and near the threshold of the classroom, and there were cobwebs in the doorframe of the restroom.Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 8VAC20-780-550-B Description: Based on document review, the center failed to ensure the emergency preparedness plan contained all required components.
Evidence:
The emergency preparedness plan did not include the following items; establishment of center emergency officer and back-up officer to include 24-hour-contact telephone numbers for each person; accommodations or special requirements for infants, toddler, and children with special need to ensure their safety during evacuation, relocation, shelter-in-place and during lockdowns; procedures to reunite children with a parent or authorized person designated to pick up the child, and a continuity of operations to ensure essential functions are maintained during an emergency.Plan of Correction: Not available online.
