Comments:
An unannounced monitoring inspection was conducted on-site March 26, 2024 from 10:46am until 1:15pm. The director was available during the inspection. There were 45 children present with 14 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Violations:
Standard #: 8VAC20-780-240-A
Description: Based on a review of staff records and interview, the center failed to ensure that staff complete the Virginia Department of Education sponsored orientation course within 90 calendar days of employment.
Evidence: The record of staff #2, hired 01/2/23, contained documentation of the orientation dated 07/15/23.

Plan of Correction: Create teacher spreadsheet with due dates of important/required criteria. Add dates to outlook calendar as a reminder before teacher requirements are due.

Standard #: 8VAC20-780-280-B
Description: Based on observation, the center failed to ensure that hazardous substances such as cleaning materials, were kept in a locked place using a safe locking method that prevents access by children.
Evidence: There was Clorox spray in an unlocked cabinet in the bathroom in the toddler classroom. The label on the Clorox stated "keep out of reach of children" and "warning."

Plan of Correction: Retrain staff on importance of locking hazardous materials.

Based on observation and interview, the center failed to ensure children's hands are washed with soap and running water after a diaper change, staff's hands are washed with soap and running water before serving food and beverages and children's hands are washed with soap and running water or disposable wipes before and after eating meals or snacks.
Evidence: 1. In the infant classroom, staff was observed to change a diaper and not wash the child's hands after the diaper change and put the child on the floor to play with a toy. The staff confirmed the child's hands were not washed.
2. Staff in the toddler room were observed to not wash their hands before serving lunch.
3. Children in the toddler room were observed to transition from the floor to the table for lunch without washing their hands.

Description: Based on observation and interview, the center failed to ensure any food from home container is clearly dated and labeled in a way that identifies the owner.
Evidence: In the toddler 3 classroom there were 3 water bottles brought from home that weren't labeled with a name or date. There was 1 water bottle from home with no date and there were 4 water bottles from home labeled with the wrong date. Administration confirmed the water bottles were brought from home and were not labeled properly.

Plan of Correction: Send message to parent to ensure bottles are labeled/staff also label.

Standard #: 8VAC20-780-560-G
Description: Based on observation and interview, the center failed to ensure any food from home container is clearly dated and labeled in a way that identifies the owner.
Evidence: In the toddler 3 classroom there were 3 water bottles brought from home that weren't labeled with a name or date. There was 1 water bottle from home with no date and there were 4 water bottles from home labeled with the wrong date. Administration confirmed the water bottles were brought from home and were not labeled properly.

Description: Based on observation and interview, the center failed to maintain a record of each child on formula to include the brand of formula and the child's feeding schedule.
Evidence: In the infant 2 classroom there were three children on formula. There was no documentation of the brand of formula for each child. Staff in the classroom reported they did not know the brand(s) of formula for each child.

Plan of Correction: Inquire with parents about formula brand and note on whiteboard in classroom.

Description: Based on observation and interview, the center failed to maintain a record of each child on formula to include the brand of formula and the child's feeding schedule.
