Comments:
An unannounced monitoring inspection was conducted today between the hours of 10:30am-1:30pm. There were a total of 35 children with seven supervising staff members present. Children were observed during bottle feeding, lunch, coloring, and engaged in group activities. A selection of staff and children records and other documents were reviewed. The physical space, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Repeat violations were found and are identified within this report.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use staff names; list staff by positions only.

Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website.

Violations:
Standard #: 8VAC20-780-130-E
Description: Based on record review, the center did not obtain documentation of additional immunizations once every six moths for children under the age of two years.

Evidence:
1. Child #1's record (Start date-01/17/2023) did not have documentation of additional immunizations. The last documentation of immunizations was dated 08/04/2023 when the child was 18 months old.

Plan of Correction: -Reach out to parents about updated shots.
-Ensure that the updates are given when they go to the doctor for checkups. Suspend care when parents don?t bring in shot records on-time.

Standard #: 8VAC20-780-210-A
Description: Based on record review and interview, the center did not ensure lead teachers met one of the lead teacher qualifications.
Evidence:
1. Staff #6 stated that Staff #4 was the lead teacher in the Infant 1 classroom. Staff #4 was the only staff member present in the classroom.
2. Staff #4's record did not contain documentation that Staff #4 met the qualifications for being a lead teacher.

Plan of Correction: -We will obtain records that qualify lead teachers before they begin. This includes paperwork from previous employers verifying the in-class experience needed to be a lead teacher.

Description: Repeat Violation

Based on observation, hazardous substances such as cleaning materials were not kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. In the Pre-K classroom, two bottles of sanitizer were located in a tall cabinet that was not locked up.

Plan of Correction: -We will make sure all cabinets that contain hazardous substances will be locked.
-Members of management will be going around to classrooms throughout the day to ensure that all cabinets are locked up.
-Set up time in which teachers have the extra time to ensure safety checks throughout the day.

Standard #: 8VAC20-780-500-A
Description: Repeat Violation

Based on observation, children's hands were not washed with soap and water or disposable wipes before eating meals.
Evidence:
1. In the Infant 2 classroom, Child #2s, Child #6's, and Child #8's hands were not washed before eating lunch.

Plan of Correction: -Washing infants? hands before lunch.
-Setting a time in the schedule where it is written for handwashing time.
-If need create a chart or a checklist that teachers are needing to check off to ensure that it is being done.

Standard #: 8VAC20-780-570-E
Description: Based on observation, prepared infant formula was not dated and labeled with the child's name.
Evidence:
1. In the Infant 1 classroom, three bottles of prepared infant formula were located in the refrigerator not labeled with the child's name and date.

Plan of Correction: -Checking bottles every morning for labels even if parents have already placed in the refrigerator.
-Having spare labels in the classroom in which can be used to label when parents forget to label their bottles.
