Comments:
An unannounced monitoring inspection was conducted on 3/28/2024 at 9:45am. There were 38 Toddlers to 5yr olds directly supervised by 10 staff. The physical plant, 4 staff records, 5 children?s records, no children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play and circle time, arriving to care, and preparing to go outside for a walk. Diapering and hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Description: Based on review and staff statements, the facility did not ensure that they maintained an injury prevention plan that was updated at least annually based on documentation of injuries and a review of the activities and services.
Evidence: reviewed the facility's records and found that it did not have documentation of an annual review or udpate.

Plan of Correction: Annual review was conducted. Report was sent via email.

Standard #: 8VAC20-780-80-A
Description: Based on review and observation, the facility did not ensure that a written record of daily attendance documents that arrival and departure of each child in care as it occurs.
Evidence: reviewed the attendance log in the preschool 1 classroom and found it documented 9 children in the classroom, but confirmed through observation and classroom teachers that there were 10 children in the room.

Plan of Correction: Corrected on the day of the inspection.

Standard #: 8VAC20-780-280-B
Description: Based on observation, the facility did not ensure that hazardous substances were kept in a locked place using a safe locking method that prevents access by children.
Evidence: observed that there was shaving cream marked "keep out of reach of children" in an unlocked lower cabinet accessible to children.

Plan of Correction: Corrected the day of the inspection - staff was reminded that these substances should remain locked and inaccessible to children.
