An unannounced inspection was conducted on 4/3/24 from 10:00 am to 12:15 pm. There were 90 children with 15 staff members. 4 staff records, 4 children's records, emergency drill logs, injury reports, medications and attendance was reviewed. Children were observed engaged in small center activities, learning about domestic and wild animals, and preparing for lunch.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Please complete the plan and date of correction for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use staff names; list staff by positions only. Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website. Standard #: 8VAC20-770-60-C-2 Description: Repeat Violation
Based on records review and staff interview, the center did not obtain results of a central registry search (CRS) for an employee within 30 days of employment.
Evidence:
1. Staff # 1's first day of employment was 2/20/23.
2. Staff # 1's CRS was submitted in November 2023. The request was bounced back due to missing information.
3. The center did not re-submit the request to this date.
4. Staff # 2's first day of employment was 1/2/24.
5. Staff # 2's CRS has not been obtained to this date.Plan of Correction: Had staff # 1 refill out CRS form since previous one kicked back to resend it. Send in staff #2?s CRS. paperwork.
Standard #: 8VAC20-780-130-A Description: Based on records review, the center did not obtain shall obtain documentation of immunizations by the first day of attendance.
Evidence: Child # 1's first day of attendance was 3/4/24. Child # 1's immunization records were not obtained by the center.Plan of Correction: Spoke with and followed up with parents of child # 1 to receive records for a file.
Standard #: 8VAC20-780-160-B Description: Based on records review, the center did not obtain acceptable forms of documentation for a Tuberculosis (TB) screening for an employee.
Evidence:
1. The TB screening for Staff # 3 contained the date of the TB screening and notes about the questions that Staff # 3 answered related to the screening.
2. The screening form did not indicate whether or not the employee had a negative TB reading.Plan of Correction: Spoke with staff # 3 to schedule and complete testing/paperwork for TB reading.

Evidence:
1. Staff # 2's first day of employment was 1/2/24.
2. Staff # 2's record did not contain of any documentation to show that Staff # 2 had completed orientation.Plan of Correction: Reviewed orientation with staff #2 and completed documentation.

Based on observation, hazardous substances were not maintain in locked place using a safe
locking method that prevents access by children.
Evidence:
1. Cleaning spray bottles were stored in a cupboard in the Transitional Kindergarten (4-5 yrs. old) classroom.
2. The cupboard was not locked.Plan of Correction: Order new lock for closet to replace broken one. Lock was broken, ordered new one
Standard #: 8VAC20-780-350-B-4 Description: Repeat Violation
Based on observation and record review, the center did not maintain the ratio of one staff to ten children in a group that included children four years of age.
Evidence:
1. At approximately 10:10am, there were 12 children with Staff # 1 in the Transitional Kindergarten (TK) room. The children were ages 4- 5 year old.
2. The child supervision record (CSR) also showed that 12 children were checked in.
2. Staff # 1 was by herself and over ratio until 10:17 am, when 3 children were taken out for a phonics class to a different room.Plan of Correction: Met with staff member, reviewed ratios, reminded staff to let management know whenever over ratio.
