Comments:
The licensing inspector conducted an unannounced complaint inspection on 03/27/2024 concerning staff-to-child ratio. Documentation was reviewed and staff interviewed. The preponderance of the evidence gathered during the investigation supported the allegation. The complaint is determined to be valid.

Please sign and complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Description: Complaint Related
Based on documentation and interview, the center did not ensure compliance with ratios of staff to children as required.

Evidence:
(Repeat Violation) On 03/27/2024, the licensing inspector entered the Infant I room at approximately 8:39 a.m. and reviewed the ?Face-To-Name Transition Sheet?. The Face-To-Name Transition Sheet documented S1 being in the classroom by herself between 7:22 a.m. and 7:43 a.m. with 10 children until S2 removed 4 children from the classroom at 7:43 a.m.
The Face-To-Name Transition Sheet documented S1 being in the classroom alone again between 7:43 a.m. and 8:00 a.m. with between 6 and 11 children when S3 removed 7 children from the classroom.
S1 acknowledged the room was out of ratio several times that morning as well as other times on other days.
S4 reviewed the Face-To-Name Transition Sheet and acknowledged the Infant I room was out of compliance with the allowed ratio of 1 staff to 5 children due to the ratio waiver being granted.

Plan of Correction: Management reviewed LCG & VA State regulations regarding ratios with all staff on (03/27/2024). The review also included the expectation that when a classroom is at max ratio teachers are to inform parents at drop off that they are at max ratio and direct families to see management. Additionally, signs have been created for all classrooms to post on their door informing families to see management before dropping off as room is at max ratio. This will ensure ratios are met at all times.

Description: Based on observation, the provider failed to fasten the protective belt securely when a child is placed in an infant seat or high chair.
Evidence:
Three infants were observed seated in highchairs. None of the protective belts were fastened.

Plan of Correction: Staff received a retrain on safety and the importance of fastening all safety belts while children are sitting in highchairs on 04/09/2024.
