A renewal inspection was conducted on December 18, 2023 from approximately 9:15 AM to 1:25 PM. There were 66 children present in the center, ranging in ages from infant to preschool, with 12 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of eight child records, six staff records, and two board member records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Please complete the plan of correction and the date to be corrected for each violation listed on the violation notice and return it to the department within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Description:	Based on a review of board member records, the center did not ensure that current board members had the required repeat background checks every 5 years. Evidence: 1. The record of board member #1 contained documentation of the results of the fingerprint-based national criminal background check update dated 7/10/2023. The last fingerprint-based national criminal background check result in record is dated 6/29/18. 2. The record of board member #2 contained documentation of the results of the fingerprint-based national criminal background check update dated 7/18/2023 The last fingerprint-based national criminal background check result in record is dated 7/02/18.

Description:	Based on observation, the center did not ensure that the surface of tables were sanitized using a spray disinfectant solution before and after each use for feeding. Evidence: 1. In the cafeteria children finished lunch and transitioned to another room. Staff did not sanitize the cafeteria tables after use.

Description:	Based on observation, the provider failed to ensure that any vehicle used by the center for transportation of children shall be kept in a satisfactory condition to assure the safety of children. Evidence: Van #1 had an expired inspection sticker dated 11/23. The administrator acknowledged that the van has been used since the inspection sticker expired.
