Technical Assistance:
Discussed the importance of placing infants on their backs for nap.

All children under the age of 5 years are required a rest time for at least 30 minutes.

Discussed transitions during your daily schedule and the need to have floating staff to ensure that your class ratios are maintained.

Some blinds in classrooms have been replaced. Please ensure that the cords of the older blinds are inaccessible to children in care.

Comments:
At unannounced Monitoring inspection was conducted today. There were a total of 124 children with 34 staff onsite. At the beginning of the inspection, the center was actively engaged in picture day. The Director was on the lower level assisting with classes who were lined up to have their pictures taken. Observation of rooms 10, 11, 12, 9 and 4 along with the infant classrooms was completed. A birthday celebration with pizza was observed in one of the classrooms. Other classes were observed transitioning from the playground to inside. Handwashing and diapering procedures were observed and transitions from classrooms to the lunch room was made. A total of 5 staff files and 8 children's files were reviewed along with medication, authorizations and allergy action plans. The previous violations from the last inspection were reviewed and the corrective actions were verified today. Violations were found today and discussed with the Director. Please provide a corrective action for each violation. This inspection occurred between 10:15am 1:20pm. Thank you for your cooperation today. Standard #: 22.1-289.035-B-2
Description: Based on staff record review, a background check clearance was not found for one staff.
Evidence:
Staff #3's last documented fingerprint clearance was in February 2018. A fingerprint search request is required every five years.

Plan of Correction: The staff member had their
background clearance
re-done and we received a new one .

Standard #: 8VAC20-770-60-C-2
Description: Based on staff record review, a background check clearance was not found.
Evidence:
Staff # 2 was hired on 08/23/2021. A Central Registry Search result has not been obtained.

Plan of Correction: Requested it to be re-done

Standard #: 8VAC20-780-280-B
Description: Based on observation of the center, hazardous substances were not kept in a locked place using a safe locking method that prevents access by children.
Evidence:
A cabinet located in Class #10 contained cleaning agents. The cabinet was found unlocked.

Plan of Correction: Talked to staff about the importance of keeping hazardous substances locked in a cabinet at all times .

Description: Based on observation of the school aged playground, resilient surfacing did not comply with minimum safety standards.
Evidence:
The sufficient resilient surfacing was not maintained under the fall zones located on the school aged playground.

Plan of Correction: Mulch and sand were ordered, and will be delivered on 4/12/2024. It will be spread on 4/13/2024 when no children are present.

Description: Based on observation of the playground, a sandbox was not covered when not in use.
Evidence:
The sandbox located on the preschool playground was not covered and was not currently being used.

Plan of Correction: Spoke to staff about assigning someone to cover the sandbox when your class is coming indoors. A new cover was ordered.

Standard #: 8VAC20-780-500-B
Description: Based on observation of one classroom, a changing table or countertop designated for diaper changing for children younger than 3 years was not maintained.
Evidence:
Class #9 has children under the age of 3 years. A diaper changing table or countertop is not used when changing diapers.

Plan of Correction: A new changing table was ordered

Standard #: 8VAC20-780-560-G
Description: Based on observation, food brought from home were not properly labeled.
Evidence:
Lunchboxes located in Class #10 were not labeled with today's date.

Plan of Correction: Reminded staff that it is mandatory all food from home Is labeled with today's date
