Comments:
A unannounced monitoring inspection was conducted today with the director and staff from 10:20 am to 11:45 am and 12:15 pm to 12:30 pm. There were 15 children present with 4 staff providing supervision. The ages of the children present ranged from 2 to 5 years. During most of the inspection the 2, 4 and 5 year old children were outside on the playground. There were no medications on site. The center does not provide transportation at this time. There have been no injuries/accidents for 2024. Three staff and six children's files were reviewed. The areas of non compliance are outlined on the violation notice.

Violations:
Standard #: 8VAC20-780-210-A-4-a
Description: Repeat violation:
Within 6 months before or 1 month after becoming a lead teacher, 24 hours of training shall be received in four areas.
Staff A, B and C are functioning as lead teachers however there was no 24 hour lead teacher training provided.
Staff A was hired on 9/11/23, staff B was hired on 11/22/21 and staff C was hired on 5/15/23 as lead teachers.

Plan of Correction: We will provide the 24 hours of lead teacher training.

Standard #: 8VAC20-780-270-A
Description: Repeat Violation:
Areas inside and outside shall be maintained in a clean, safe and operable condition. Two to four children were observed hanging out of the window in the blue classroom. This classroom serves 3 year old children. The window was open and there was no screen in the widow to prevent a child from hanging out of the window.

Plan of Correction: We will not open the window in that room until we can replace the window screen.

Standard #: 8VAC20-780-280-B
Description: Hazardous substances shall be kept in a locked place to prevent access by children. In the "big classroom" there were 16 containers of hazardous substances unlocked and accessible to children on a table in the classroom. On a shelf unlocked in the 3 year old room was shaving foam. In the unlocked laundry room on the floor was a large container of paint.

Plan of Correction: The hazardous substances were locked.

Standard #: 8VAC20-780-280-C
Description: Insecticides shall not be stored in areas used by children. A container of insecticide was observed sitting unlocked and accessible to the children in the big classroom.
