A monitoring inspection was completed on April 9, 2024 from 10:06 am until 1:18 pm at the center. There were 62 children present, ranging in ages from infants to four years with 12 staff including administrative supervising. The inspector and home office representative completed observations of the children in care engaged in circle time activities with staff, diapering by staff, participating in active play in the classroom, participating in outdoor playground activity, completing bathroom breaks/handwashing, transitioning indoor to prepare for lunch and eating lunch. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, medications, programming, special care and emergencies and nutrition. A total of 8 children records and 5 staff records were reviewed.

Standard #: 8VAC20-780-270-A Description: Based on observations and photographs, the center did not ensure all areas and equipment of the center inside and outside, shall be maintained in a clean, safe, and operable condition.
Evidence: The following were observations at the center during the inspection:
1. In the CuddleBugs classroom, there were multiple areas of peeling paint on the walls.
2. In the Caterpillar 1 classroom, there were multiple areas of peeling paint on the walls.
3. In the Caterpillar 2 classroom, there were multiple areas of peeling paint on the walls, and a visible hole in the wall.
4. In the Caterpillar 2 classroom, there was one electrical outlet with a visible crack on the side and a yellow bin containing toys with two visible cracks on the sides with sharp points exposed.
5. In the Caterpillar 1 and 2 classrooms, there were multiple area rugs with curled corners and unsecured causing a tripping hazard.
6. In the Butterfly 1 and 2 classrooms, there were multiple areas of peeling paint on the walls.
7.. In the School Age classroom, there were multiple areas of peeling paint on the walls.
8. The outdoor sandbox wooden barrier log was loose and separate from the base causing a gap for a safety hazard.
9. There were multiple protruding nails on the wooden fence areas surrounding the air condition units on the outdoor playground area.

Standard #: 8VAC20-780-280-B Description: Based on observation and photograph, the center did not ensure all hazardous substances are kept in a locked place using a safe locking method that prevents access by children.
Evidence: 1. In the center's School age classroom, there was observations of a unlocked cabinet containing a large bottle of sunscreen spray with the labels "keep out of reach of children" "warnings" and "flammable".
