Comments:
An unannounced monitoring inspection was conducted on April 10, 2024 between the hours of 9:00am-11:15am. There were a total of 38 children with seven supervising staff members present. Children were observed during breakfast, handwashing, diaper changing,engaged in group activities and outdoor play time. A selection of staff and children records, medications and other documents were reviewed. The physical space, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Violations:
Standard #: 8VAC20-780-160-A
Description: Based on record review and interview, each staff member did not submit documentation of a negative tuberculosis(TB) screening at the time of employment.
Evidence:
1. Staff #3's record (Date of hire-02/22/2024) did not contain documentation of a negative Tb screening. Staff #1 confirmed that Staff #3 had a TB screening, but could not locate documentation at the time of the inspection.
2. Staff #4's record (Date of hire-02/19/2024) and Staff #5's record (Date of hire-01/22/2024) did not contain a negative TB screening completed within the last 30 days of the date of employment. Staff #4's negative TB screening was dated 02/22/2024 and Staff #5's negative Tb screening was dated 01/29/2024.

Plan of Correction: Moving forward, all new staff will submit TB screenings at the time of employment or within the last 30 calendar days of the date of employment. Staff will submit TB screenings in a timely manner and the record will be documented.

Standard #: 8VAC20-780-60-A
Description: Based on record review, children's records did not contain required information.
Evidence:
1. Child #3's record (Start date-12/27/2022) did not contain documentation of the child's updates and confirmation of up to date information in the child's record.
2. Child #4's record (Start date-10/19/2021) did not contain documentation of the child's updates and confirmation of up to date information in the child's record.

Plan of Correction: Management will have the parents to review the records to provide up to date information/annual review.

Standard #: 8VAC20-780-240-I
Description: Based on record review and interview, documentation of orientation training was not kept by the center.
Evidence:
1. Staff #3's record (Date of hire-02/22/2024) did not contain documentation of orientation training.
2. Staff #7 confirmed that Staff #3 completed orientation training, but could not locate the document at the time of the inspection.

Plan of Correction: Staff orientation will be completed and the record will be documented.

Standard #: 8VAC20-780-500-A
Description: Based on observation, children' s hands were not washed with soap and running water or disposable wipes after eating meals or snacks.
Evidence:
1. In the Infant 1 classroom, Staff #6 did not wash one child's hands after eating a meal.

Plan of Correction: The staff will be retrained on handwashing procedures.
