Technical Assistance:
Discussed ensuring that medication listed on written authorization must match the name of the medication given to the center and if written authorization lists a brand name medication, the generic cannot be given.

Discussed ensuring accurate attendance when classrooms are split and occupying different locations of the building.

An unannounced monitoring inspection was conducted on 03/25/2024 from 9:15 a.m. to 12:25 p.m. with the Director. There were 66 children in care, ranging in age from three months to five-years-old, supervised by 14 staff. The children were observed in circle time and going outside. Five child records and five staff records were reviewed. Eight medications and authorization forms were reviewed, and the center has one staff current in Medication Administration Training (MAT). Required postings were observed. The attendance, menu, and emergency drill log were reviewed. The first aid kit, flashlight, and battery-operated radio were observed. The most recent Fire Inspection on file was dated 02/09/23. Lunch served was chicken sandwich, pears, green beans, and milk. Areas of non-compliance are identified in the violation notice. Standard #: 8VAC20-780-70
Description: Based on a review of four staff files, the center did not obtain documentation that two or more references as to character and reputation as well as competency were checked before employment.

Evidence: The record for Staff #4, with a start date of 2/26/24, did not contain documentation of two references at the time of inspection.

Plan of Correction: Employee references are complete

Standard #: 8VAC20-780-80-A
Description: Repeat Violations Based on review of the written attendance record, the center did not ensure they maintained a written record of daily attendance for each group of children documenting arrival and departure of each child in care as it occurs.

1. In the three-year class there were 16 children present, the written record reflected 15.
2. The toddler class had no written record of attendance when they were at the bounce house. Staff stated that the attendance was in their classroom and that they did not bring the attendance with them.

Plan of Correction: Re-trained all staff regarding roll sheets and clocking in the children as they walked into the classroom. Keep clipboards on them at all times.

Standard #: 8VAC20-780-240-B
Description: Based on review of five staff records, the center did not have documentation that staff completed orientation training no later than seven days of the date of assuming job responsibilities.

Evidence: The record for Staff #4, with a start date of 2/26/24, did not contain documentation of completed orientation training at the time of inspection.

Plan of Correction: This employee has completed orientation.

Standard #: 8VAC20-780-270-A
Description: Based on observation, it was determined that not all areas of the center inside and outside were maintained in a clean, safe, and operable condition.

Evidence: On the three-year and up playground warped boards on the fence were observed along with nail pops near the ground posing an impalement and scratch hazard. The playground had 1- ? to 3-inch gaps in the seam of the artificial grass in the area where children play, posing a trip hazard.

Plan of Correction: Contacted the people that installed the playground equipment for repair.
