A renewal inspection was conducted on March 14, 2024 from approximately 1:28 PM to 5:30 PM. There were 101 children present in the center, ranging in ages from infant to school-age, with 21 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 10 child records, 10 staff records and four board members were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the plan of correction and the date to be corrected for each violation listed on the violation notice and return it to the department within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names, list staff by positions only.

Standard #: 8VAC20-780-270-A Description: Based on observation, the center did not ensure areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
Evidence:
1. In room #11, one toilet paper holder was broken and off the wall.

Standard #: 8VAC20-780-500-B Description: Based on observation, the center did ensure that a leakproof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal.

Standard #: 8VAC20-780-510-I Description: Based on review of medications and medication documentation, the center did not ensure that prescription medication was administered as required.
Evidence:
Medication authorizations were reviewed for child B. The center did not obtain written authorization from parent or guardian to administer prescription medication.
