Comments:
An unannounced complaint investigation was conducted on April 2, 2024. Staff Interviews were conducted, records reviewed, and video observations made in the Infant classroom in response to a complaint alleging staff qualifications and training and daily activities for infants.The preponderance of evidence does support the allegations and the complaint has been deemed valid.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use staff names; list staff by positions only.


Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website.

Standard #: 8VAC20-780-370-5
Complaint related: No
Description: Based on video observation and staff interview, staff did not provide awake infants not playing on the floor or ground a change in play space at least every 30 minutes or more often as determined by the individual infant's needs.
Evidence:
1. On 03/25/2024, at approximately 9:17am, Child #1(2 months) was placed in a bouncy chair by Staff #2. Child #1 was removed from the bouncy chair at approximately 10:43am.
2. On 03/25/2024, Child #1 did not receive a total of 30 minutes each day of tummy time to facilitate upper body strength and to address misshapen head concerns.
3. Staff #1 confirmed that Child #1 was placed on their stomach for approximately five minutes, but because their face turned red, stopped tummy time. Staff #1 confirmed that tummy time for Child #1 was not done for the rest of the time in care.

Plan of Correction: We will provide tummy time to all age-appropriate children for at least 30m a day. We will also document and mark it in the messaging app for the parents to know how long and when.
We will remove the bouncy seat.
We will rotate children from one play area to another every 30m.

Description: Based on video observation, children's hands were not washed before and after eating meals or snacks.
Evidence:
1. On 03/25/2024, Child #2 (13months) and Child #3 (11months) did not wash their hands with soap and running water or disposable wipes before and after eating a meal.
2. On 03/25/2024, Staff #1did not wash their hands with soap and running water before feeding a bottle to Child #1.
3. On 03/25/2024, Staff #2 did not wash their hands with soap and running water before preparing or serving food or beverages and before and after a diaper change.

Plan of Correction: Washing infants? hands before lunch.
Setting a time in the schedule where it is written for handwashing time.
Staff washing hands all the time, before serving meals / bottles, after serving meals / bottles. When coming in contact with bodily fluids, etc.
If need create a chart or a checklist that teachers are needing to check off to ensure that it is being done.

Complaint related: No
Description: Based on video observation, one bottle fed infant was not held when being fed.
Evidence:
1. On 03/25/2024, at approximately 9:45am, Staff #1 was observed feeding Child #1(2months) a bottle while Child #1 was laying in a bouncy.

Plan of Correction: We will retrain teachers of where to properly feed infants.
We will feed infants only in staffs arms.
